ABSTRACT

Surgical tumor excision is a cornerstone of primary cancer treatment, and persistent postoperative pain is common after most surgical procedures. Surgical trauma and pain can cause inflammation, hypothalamic-pituitary axis activation, and sympathetic system over-reactivity. These factors, acting either separately or in combination, affect anti-tumor immunity and promote postoperative immunosuppression. Effective management of perioperative pain may help to modulate the unwanted consequences of the stress response on the immune system and improve outcomes. Based on the recent literature and clinical studies, experts from The Society of Oncological Anesthesia and Analgesia, Chinese Anti-Cancer Association have developed this consensus on the perioperative pain management of cancer patients in China (2020 edition). This consensus aims to provide evidence-based clinical recommendations for the acute pain management of cancer patients postoperatively. It does not apply to chronic pain or pain caused by cancer.

ABSTRACT

OBJECTIVE@#To observe the analgesic effect of auricular point sticking therapy during the perioperative stage in the patients with partial lung resection.@*METHODS@#A total of 92 patients with partial lung resection were randomized into an auricular point group (31 cases, 1 case dropped off), the sham-auricular point group (30 cases) and a medication group (31 cases, 1 case dropped off). The routine medication for analgesia was provided in all of the three groups. In the auricular point group, 1 day before operation, the auricular point sticking therapy was applied at shenmen (TF@*RESULTS@#In 8, 16, 24, 48 h and 72 h after operation, VAS scores in the auricular point group were lower than those in the sham-auricular point group and the medication group separately (@*CONCLUSION@#Auricular point sticking therapy relieves perioperative pain, shortens analgesic time, releases anxious and depressive emotions and reduces postoperative adverse reaction in the patients with partial lung resection. The analgesic mechanism is probably related to the increase of plasma concentration of β-endorphin.

ABSTRACT

Objective To find out the current situation and related factors of perioperative pain management for the patients with fracture among the top three hospitals in our region.Methods A general information questionnaire was made,to survey the the status and the influencing factors of top hospital nursing work of fractures patients with perioperative pain management.The researchers label the questionnaire with the method of digital label,and the data collected are kept secret,and the statistical method is used to evaluate it.Finally,the conclusion is drawn.Results The working life of the nursing staff,nursing education,nursing staff after underwent fracture perioperative pain knowledge of continuing education is the mainly factors to affect patients surfing with fracture,perioperative pain management cycle of practice the three most important factors.Conclusion Three tertiary hospitals in the area fracture nursing of pain management need to be strengthened.we strengthen fractures patients with perioperative pain management cycle practice by training of clinical nursing staff.

ABSTRACT

Objective@#To investigate the effect of preoperative use of paracetin sodium on epidural pain and coagulation in patients with epidural hematoma.@*Methods@#80 brain trauma patients with epidural hematoma underwent surgery were selected, and they were randomly divided into two groups according to the digital table, 40cases in each group.30min before surgery, the observation group was given parecoxib sodium, and the control group was treated with the same volume of 0.9% saline.Then, the changes of the visual analogue scale (VAS score) and PCIA compression times were compared between the two groups at 6h, 12h, 24h and 48h after operation.The coagulation changes were observed.@*Results@#6h, 12h, 24h and 48h after operation, the VAS scores of the pain in the observation group were (4.1±0.3)points, (4.0±0.2)points, (3.0±0.3)points and (2.3±0.3)points, respectively, which were lower than those in the control group(t=17.541, 3.508, 7.589 and 28.284, all P<0.05).6h, 12h, 24h and 48h after operation, the PCIA times in the observation group were (1.9±0.4)times, (1.8±0.3)times, (1.1±0.2)times and (0.7±0.1)times, respectively, which were lower than those in the control group (t=4.939, 3.795, 12.279 and 16.000, all P<0.05). The PT, TT, APTT and Fib between the two groups had no statistically significant differences (t=0.407, 0.000, 1.491 and 0.331, all P>0.05).@*Conclusion@#Preoperative use of parecoxib sodium can effectively reduce the perioperative pain in patients with epidural hematoma, and it has no effect on coagulation function with high safety.

Introducción: Como anestesiólogos, tenemos la obligación de proporcionales a nuestros pacientes una adecuada analgesia peri operatoria e inhibir la respuesta orgánica al estrés anestésico quirúrgico. En la cirugía cardiovascular esta respuesta proviene de dos hechos fundamentales: el trauma tisular y la circulación extracorpórea. Objetivos: Explicar las principales causas del dolor perioperatorio en la Cirugía Cardiovascular, así como la evolución en las técnicas utilizadas para su control. Desarrollo: Múltiples han sido las técnicas utilizadas para el control del dolor, en busca de la mejor opción. En los últimos años la utilización de las técnicas de anestesia _analgesia epidural e intratecal ha incrementado y a pesar de las controversias existentes, los beneficios relacionados con las mismas son superiores a los riesgos reportados. Conclusiones: La calidad de la analgesia que se logra con métodos anestésicos espinales ha resultado valida en cirugía cardiovascular para lograr una rápida recuperación e incorporación del enfermo a su vida familiar y laboral.


Like anesthesiologists and out of a sense of duty, we must to provide to our patients of a appropriate perioperative analgesia and to inhibit the organic response to surgical anesthetic stress. In cardiovascular surgery this response comes from two fundamental facts: tissue traumatism and extracorporeal circulation. Aims: To explain the main causes for its control. Development: Many have been the techniques used to pain control in search of a better option. In last years the use of anesthesia-analgesia techniques (epidural and intrathecal) has increased, despite the present controversies, the benefits related to both are superior to reported risks. Conclusions: Anesthesia quality with spinal anesthetic methods has been valid in cardiovascular surgery to achieve a fast recovery of patient and its incorporation into family and working life.
